SUMMARY:
A well-established company located in Johannesburg North is looking for a Cardiovascular Case Manager to join their team!

POSITION INFO:
Purpose of the Role:
Responsible for the ongoing development and management of the Disease Management Programme, ensuring optimal patient care, risk management, and continuous process improvement.

Requirements:

Qualifications: Registered Nurse, registered with the South African Nursing Council.
Experience: 1-2 years in medical aid or managed healthcare preferred; experience in disease management is advantageous.
Technical Knowledge: Strong understanding of chronic disease treatments, clinical protocols, case management, and prescribed minimum benefit guidelines.
Skills: Excellent communication (written and verbal), report writing, time management, problem-solving, and computer literacy.

Key Responsibilities:

Develop and distribute educational materials for patients.
Continuously enhance Disease Management Programmes.
Conduct retrospective reviews of related claims.
Develop and maintain clinical protocols, including new drug assessments.
Collaborate with healthcare professionals within the organization.
Liaise with pharmaceutical companies.
Compile monthly reports on program progress and cost-saving opportunities.
Manage all related hospital admissions.
Stay updated on benefit changes.
Deliver educational presentations to employer groups.
Assist with interdepartmental queries and perform ad hoc duties.
